Berdasarkan ketentuan dalam Permendagri Nomor 79 Tahun 2018 tentang Badan Layanan Umum Daerah bagian kesatu pasal 3, pengelolaan SDM BLUD diarahkan agar mendukung fleksibilitas dan akuntabilitas pelayanan publik. Ketiganya memikul tugas dan tanggung jawab yang berbeda namun saling terkait untuk memastikan layanan publik yang berkualitas. Pejabat pengelola dari tenaga profesional diangkat maksimal 5 tahun dan dapat diperpanjang satu kali periode, dengan batas usia pengangkatan kembali maksimal 60 tahun.
